| Beginning April 14, 2003 we must provide patients with the information
regarding the HIPAA regulations. Patients must also sign a form (Acknowledgement of Receipt) that acknowledges
the receipt of such information. The Notice of Privacy and the accompanying
Acknowledgement of Receipt is attached below. Please feel free to print out both
forms and bring the Acknowledgement to the office when you come to your next
